Hip Pain in Joint
Pain can arise from the hip joint itself or from the structures around the hip joint. The hip joint is a weightbearing ball and socket joint.
Pain can be in the front – in the groin, lateral to the side or posterior, in the rear or far posterior.
Anterior hip pain is usually deep in the groin and can result from arthritis of the hip. The treatment is usually a conservative treatment with therapy, medications and injections. Surgical treatment in late cases is by a total hip replacement.
A second condition is a labral tear, usually diagnosed by an MRI arthrogram. The treatment is done by therapy, medication and injections. Surgery in the back can be done by debridement or repair of the lesions, usually arthroscopically.
Stress fracture is usually diagnosed by an MRI. Early diagnosis is important before the fracture is displaced. Usually, it requires a fixation or replacement if it is faint.
Avascular necrosis is due to death of a segment of the bone when blood supply of the femoral head is interrupted. Treatment is usually by decompression, a stem cell injection or drilling if it is early. Early cases are diagnosed by MRI and severe cases are diagnosed by x-rays. The treatment is usually a total hip replacement. Intermediate cases are usually treated by vascularized fibular graft or other conditions.
Lateral hip pain is usually due to trochanteric bursitis. The treatment is usually conservative with therapy, anti-inflammatory medications and injections. Surgical treatment is extremely rare with excision of the bursa.
Posterior hip pain is due to piriformis syndrome. The treatment is usually physiotherapy, stretching, anti-inflammatory medications and injections. Surgery is extremely rare with severe cases that do not improve with conservative treatment. Must know that piriformis syndrome irritates the sciatic nerve.
Far posterior hip pain is usually due to SI joint and lower spine conditions. SI joint problems can only be diagnosed by injection of the SI joint with improvement in the pain level of the patient.
